The Physician Case Manager monitors individuals receiving medication services and collaborates with the treatment team to create and track measurable goals. Responsibilities include linking patients to community resources, developing safety plans, and managing clinical documentation.
Candidates must possess at least an Associate degree in humanities with behavioral health experience or a 4-year degree in a related field. Experience in behavioral health programs and the ability to communicate effectively are required.
Physician’s Case Manager (PCM) is responsible for monitoring all persons served receiving medication services only. PCM collaborates with the treatment team and Director to best meet the needs of persons served and quality improvement measures of the program. The PCM provides case management services to persons served receiving only medication services.
• PCM must have a minimum Associates degree in the humanities combined with experience/training in a behavioral health program or a 4-year degree in a related field with experience.
