Complete comprehensive nursing assessments and develop individualized plans of care. Perform admission, routine, and follow-up visits based on physician orders and patient needs. Monitor patient progress and identify changes in condition, communicating findings promptly with physicians and the interdisciplinary care team. Educate patients and caregivers on disease management, medications, treatments, and self-care. Coordinate care with therapists, aides, physicians, and other healthcare professionals to ensure seamless patient care. Accurately complete clinical documentation in a timely manner. Maintain compliance with agency policies, HIPAA regulations, Medicare Conditions of Participation, and professional nursing standards. Deliver compassionate, patient-centered care that aligns with Elara Caring's mission and values. What You'll Need Current, unrestricted Registered Nurse (RN) license in the state of practice. Minimum of one (1) year of nursing experience in an acute care, skilled nursing, or clinical care setting. Previous home health experience is preferred, but not required. Experience with HomeCare HomeBase (HCHB) is a plus. Strong assessment, communication, and critical thinking skills. Reliable transportation, valid driver's license, and current automobile insurance. Ability and willingness to travel throughout the assigned service area. Physical Requirements: Sit, stand, bend, lift, and move intermittently and be able to lift 50–100 lbs. Requirements

Candidates must hold a current, unrestricted state RN license and have at least one year of clinical nursing experience. A valid driver's license, dependable vehicle, and the ability to travel 50% of the time are required.
