Serve as an empaneled primary care provider delivering comprehensive, relationship-based care to diverse patient populations. Collaborate with an interdisciplinary team to manage chronic conditions, perform clinical evaluations, and develop longitudinal care plans.
Requires a current unrestricted NP license in Oregon, a DEA license, and BLS certification. Candidates should have experience in complex primary care and a commitment to serving underserved communities.

Wallace is seeking a dedicated Nurse Practitioner to serve as an empaneled primary care provider for new and established patients, delivering comprehensive, relationship-based care in a collaborative, team-based clinic setting.
