Use Clover Assistant and clinical data to guide proactive, personalized care.
Help prevent avoidable emergency room visits and hospitalizations.
Participate in a shared on-call rotation to ensure members have continuous support.

Who you are Certified Physician Assistant, PA-C.
Located in or near Atlanta, Georgia.
Able to travel locally for in-home visits.
At least 1 year of PA experience in a clinical setting.
Active DEA registration.
Current Basic Life Support certification.
Comfortable managing chronic illness and medically complex patients.
Confident working with autonomy while collaborating with physicians and care teams.
Compassionate, grounded, and able to build trust quickly with patients and families.
Organized, responsive, and comfortable using technology to support care delivery.
Home care, house call, geriatrics, complex care, or value-based care experience is a plus.

Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.
A reasonable estimate of the base salary range for this role is: $140,000—$165,000 USD
