Position Summary

The Hospital Based Certified Nurse-Midwife (CNM) Night Shift provides comprehensive reproductive, obstetric, and immediate postpartum care to adolescent and adult patients, with a primary focus on intrapartum services delivered during overnight hours. The position primarily serves FQHC patients delivering at the health center’s partner hospital and supports continuous, high-quality maternity coverage. The night shift CNM practices within the scope of nurse‑midwifery as defined by Massachusetts law, hospital bylaws, and approved clinical privileges. Care is delivered within a collaborative model that includes consultation, co‑management, or referral to physician partners based on patient acuity and clinical need. The role ensures safe, evidence‑based, and equitable maternity care for medically and socially complex populations, advancing the FQHC mission to reduce disparities in maternal outcomes. The night shift CNM collaborates closely with obstetricians, hospitalists, nursing staff, and interdisciplinary teams to manage triage, labor progression, vaginal deliveries, and immediate postpartum care. In partnership with the Chief of OB/Gyn, certified nurse‑midwives, obstetricians, and the Chief Medical Officer, the night shift CNM contributes to the development, review, and implementation of clinical protocols, quality standards, and performance improvement activities related to nurse‑midwifery and obstetric care. PRINCIPAL DUTIES AND RESPONSIBILITIES: Provides direct patient care services within area of expertise. Assess, plans, documents and evaluates patient needs and treatments. Ensure proper follow up. Has practice privileges and attends deliveries at local community hospital. All certified nurse-midwives have a current, signed clinical practice agreement for medical consultation, collaborative management, or referral with an active Medical Staff member of BNHC who has full obstetrical privileges at the delivery hospital. Must be available for attending deliveries, in-patient admissions, c-section assists and telephone triage based on hospital coverage schedule. Organizes and conducts routine chart review and case management discussions with collaborating physician of all obstetrical clients. Manages the care of women with normal pregnancies during preconception, prenatal, childbearing and postpartum period. Manage diagnostic and therapeutic regimens for minor acute and chronic gynecological illness and seeks consultation when appropriate. Co-manages high-risk pregnancies and pregnancies with complications with consultant obstetrician and perinatologist. May provide comprehensive outpatient services on a periodic or as‑needed basis, including prenatal and postpartum care, family planning services, routine gynecological examinations, and management of minor acute gynecologic conditions, in accordance with health center policies and in collaboration with obstetrician‑gynecologists. As assigned or as clinically appropriate, provides elements of primary care and case management within the nurse‑midwifery scope of practice, including patient evaluation, assessment, treatment, and referral. When supporting ambulatory services, care emphasizes health promotion, patient education, and disease prevention consistent with the health center’s care model. Initiates follow-up and specialty referrals for those clients needing further work-up and/or monitoring of a current situation. Provides assessment, education and management of comprehensive family planning care including contraception, natural family planning, and pregnancy testing. Educates patients so that they develop an understanding of the importance of following prescribed therapeutic regimens during pregnancy and postnatal period. Coordinates the referrals to early pregnancy classes, childbirth classes, breastfeeding and parenting classes. Collaborates in the development and revision of department policies, procedures, and protocols. Participates in discussion with other clinical and administrative staff around the development of goals, objectives and plans the resolution of problems in the perinatal health services. Maintain continuity with BNHC prenatal panels when feasible; communicate handoffs and delivery summaries to clinic teams. Coordinate postpartum plans, including contraception counseling and scheduling of postpartum follow-up. Partner with social work, behavioral health, lactation, and community health workers to address social determinants of health. Adhere to BNHC, hospital, and departmental policies, clinical guidelines, and safety bundles (e.g., hemorrhage, hypertension). Participate in quality improvement initiatives, peer review, and morbidity & mortality conferences. Ensure timely, accurate documentation and coding in compliance with regulatory and billing requirements. Collaborate effectively with nursing, OB/GYNs, anesthesiology, pediatrics, and interdisciplinary colleagues and support staff. Demonstrate professionalism, reliability, and respectful communication with all colleagues and patients. Contribute to a positive team culture and support cross-coverage models that protect both L&D safety and clinic access. Qualifications MINIMUM SKILLS AND KNOWLEDGE REQUIREMENTS: Current Massachusetts licensure for R.N. in expanded role. Certification eligible as Certified Nurse-Midwife. Federal and State licensure for prescriptive writing privileges. CPR/BLS certified WORKING CONDITIONS: Works in busy community health center in downtown business district and local community hospital. Long hours frequently required. Must be available evenings and weekends, with short notice, to represent the interests of the health center. 24-hour hospital coverage required. PHYSICAL REQUIREMENTS: Physical demands require lifting small pieces of equipment, patients, and/or boxes not to exceed 50 pounds. Mostly standing and walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversation with and/or without assistive audio devices.

Requirements

Must hold a current Massachusetts RN license in an expanded role and be certification eligible as a Certified Nurse-Midwife. Requires federal and state prescriptive writing privileges and CPR/BLS certification.
