Provides comprehensive primary care within Nuvia Medical’s value-based care model.
Works with care coordination and quality teams to improve outcomes and close gaps in care.
Essential Duties & Responsibilities
Deliver full-spectrum outpatient primary care.
Manage a defined patient panel in a value-based care environment.
Close gaps in care with quality and care coordination teams.
Ensure compliance with HEDIS quality measures.
Promote appropriate utilization of healthcare services.
Develop individualized care plans for complex patients.
Participate in population health initiatives.
Review clinical dashboards and implement improvements.
Educate patients and families on disease prevention.
Document encounters accurately in the EHR.
Participate in interdisciplinary meetings and peer review.
Support Nuvia’s culture of excellence and accountability.
Requires an MD or DO degree with Board Certification or Eligibility in Family Medicine. Must possess an active Florida medical license and DEA registration.
