Conduct daily rounds and provide acute and transitional care for patients in an Inpatient Rehabilitation Facility. Collaborate with interdisciplinary teams to manage comorbidities and coordinate safe discharge planning.
Must hold a valid Nurse Practitioner or Physician Assistant license, board certification, and a Master's degree in Nursing or Physician Assistant Studies. Strong communication, leadership, and organizational skills are required.
Daily Rounding: Conduct structured daily rounds on assigned IRF patients, performing focused assessments, monitoring rehabilitation progress, and addressing acute and subacute medical needs. Medical Management: Manage comorbidities and complications common in the IRF population, including post-stroke recovery, orthopedic post-op care, traumatic brain injury, cardiac conditions, and neurological disorders. Interdisciplinary Collaboration: Partner with physiatrists, physical therapists, occupational therapists, speech-language pathologists, case managers, and nursing staff to develop and execute coordinated, patient-centered care plans. Discharge Planning Support: Actively participate in discharge planning, including assessment of functional status, coordination with case management, and communication with receiving facilities or home health agencies. Order Management: Write and manage orders for medications, diagnostics, therapies, and consults; respond promptly to nursing and therapy team communications throughout the day. Urgent and Emergent Response: Evaluate and manage acute changes in patient condition, initiate appropriate interventions, and determine when escalation or transfer is warranted. Documentation and Compliance: Maintain thorough, timely documentation in the EMR consistent with IRF regulatory standards, including required physician/APP attestation requirements and CMS compliance guidelines. Patient and Family Communication: Communicate regularly with patients and families regarding medical status, goals of care, rehabilitation expectations, and discharge planning. Quality and Outcomes: Participate in quality improvement initiatives focused on reducing hospital transfers, improving functional outcomes, and supporting IRF performance benchmarks.
Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field Masters Degree in Nursing or Physician Assistant Studies Strong leadership, interpersonal, and problem-solving skills preferred. Must possess excellent written and oral communication skills Must possess traits of punctuality, organizational skills, and ability to function within a team. This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required. Pay Range USD $50,000.00/Yr. - USD $100,000.00/Yr.
