Coordinate behavioral health services for children in the child welfare system, including reviewing assessments and managing inpatient discharge planning. Collaborate with multidisciplinary teams and Medicaid providers to remove barriers to care and ensure timely service referrals.
Requires a Bachelor's degree in social work, counseling, psychology, or a related field, with a Master's degree preferred. Must possess a valid driver's license and pass criminal background and drug screenings.
Monitor timeliness and coordination of behavioral health services. Review the recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant records related to child well-being and assist with service referral needs. Ensure Comprehensive Behavioral Health Assessments, Suitability Assessments, and other relevant behavioral health documentation are uploaded to FSFN and filed with Children’s Legal Services as required. Respond and assist with the management of inpatient notifications. Participate in discharge planning from inpatient behavioral health care facilities or crisis stabilization units, including coordinating and monitoring post-discharge follow-up recommendations. Work with the Case Management Organizations (CMO’s) and service providers to remove barriers to services. Respond to requests for behavioral health information from Independent Living transition plans. Assist with coordination of Multi-disciplinary Team Staffings (MDT) to determine level of care recommendations and obtain authorization for services when necessary. Meet with Medicaid behavioral health service providers periodically to enhance communication and reduce barriers to services. Monitor email on daily basis; and circulates pertinent information to unit staff members Perform other duties as assigned to support departmental and agency goals, as needed and appropriate to the position. Working HOurs Children’s Network of Hillsborough’s standard business hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. This position is an in-office based position that averages 40 hours per week. Work hours may vary outside of these standard hours, including evenings and weekends, as needed to meet operational demands. All hours worked will be compensated in accordance with the Fair Labor Standards Act (FLSA) and agency policies, including applicable overtime and other compensation requirements. Travel Travel within and outside of Hillsborough County is required. Training All employees are required to complete fifteen (15) hours of training annually, which may be completed online or in a classroom setting. Employees who hold a credential or license relevant to their position must ensure that all training and continuing education requirements are met to maintain their credential or licensure. Supervisory Responsibilities This position does not have supervisory responsibilities Qualifications Education and Experience Bachelor's degree required, with a preference for a Master's degree, from an accredited institution in social work, counseling, human services, psychology, or a related field. Official transcripts are a mandatory requirement Knowledge of Child Welfare preferred Strong problem solving and decision-making skills. Requires the ability to establish and maintain positive working relationships with others. Requires a working knowledge of mental health and other co-occurring disorders. Skills and Abilities Strong organizational and time management skills with the ability to coordinate multiple tasks, maintain accurate records, and ensure compliance with FSFN documentation and Children’s Legal Services requirements. Excellent communication and interpersonal skills with the ability to collaborate effectively with multidisciplinary teams, service providers, Medicaid behavioral health providers, and Case Management Organizations to reduce barriers to services and support positive outcomes for children and families. Strong professional writing skills with the ability to prepare, review, and maintain accurate documentation, case notes, reports, and correspondence in a clear, concise, and timely manner. Ability to interpret and explain behavioral health information, policies, procedures, and service recommendations to diverse audiences in a professional and respectful manner. Demonstrated ability to actively listen, gather relevant information, and communicate sensitively and effectively in emotionally challenging situations while maintaining confidentiality and professionalism. Proficiency in Microsoft Office applications, email communication, and electronic documentation systems required to support daily operations and information sharing. MATHEMATICAL SKILLS Ability to apply basic mathematical concepts and analytical skills to collect, review, and interpret data related to behavioral health services, service utilization, and case management activities. Ability to prepare and maintain accurate records, track timelines, monitor service coordination activities, and compile statistical or performance-related information with attention to detail and accuracy. Ability to calculate dates, monitor deadlines, and review information for completeness and compliance with established policies, procedures, and reporting requirements. Demonstrated ability to analyze information, identify discrepancies, and support data-informed decision-making in a fast-paced professional environment. REASONING ABILITY Ability to exercise sound judgment and apply critical thinking skills in assessing behavioral health service needs, coordinating care, and resolving barriers to services. Ability to analyze and interpret assessments, reports, policies, and related documentation to make informed recommendations and support appropriate service coordination. Demonstrated ability to identify problems, evaluate potential solutions, and make timely decisions in complex and fast-paced situations while maintaining professionalism and confidentiality. Ability to prioritize multiple responsibilities, adapt to changing circumstances, and work collaboratively with multidisciplinary teams, service providers, and stakeholders to support positive outcomes for children and families. Physical Demands The physical demands of this position are generally moderate. The role may require extended periods of sitting, standing, and using a computer. Occasional lifting of office supplies, files, or materials up to 25 pounds may be required.
