We are seeking a motivated and compassionate Licensed Social Worker (MSW) to join our home health and hospice team.
We are seeking a dedicated and empathetic Licensed Social Worker to join our Home Health / Hospice team. The Social Worker provides psycho-social assessments, emotional support, care coordination, and community resource planning for patients and families facing acute, chronic, or end-of-life conditions.
This role is an essential part of our interdisciplinary care team.
Conduct comprehensive psycho-social assessments Develop and implement individualized care plans Provide emotional support and counseling to patients and families Assist with advance care planning and end-of-life decision support Connect patients and families to community resources and financial assistance programs Participate in interdisciplinary team meetings Maintain accurate and timely clinical documentation Support crisis intervention and bereavement services (hospice) Ensure compliance with federal, state, and agency regulations
Master’s Degree in Social Work (MSW) from an accredited program Current state licensure Minimum [1 year] experience in healthcare, home health, or hospice preferred Strong communication and documentation skills Ability to work independently in a home-based setting Valid driver’s license and reliable transportation
Experience in hospice or palliative care Knowledge of Medicare/Medicaid regulations Bilingual skills (if applicable) Benefits (For Part-Time Employees)
