The RN coordinates acute patient care delivery within the patient's home environment using the nursing process to assess, plan, and evaluate care. They act as a patient advocate and collaborate with multidisciplinary teams to ensure safe transitions and prevent complications.
Candidates must be a graduate of an accredited nursing school with a valid Texas or Compact RN license and a current CPR certification. A minimum of two years of acute care nursing experience and a valid driver's license are required.
