Provide essential support to Case Planners by coordinating services, maintaining documentation, and engaging with high-risk families. Responsibilities include escorting families to service sites and participating in clinical diagnostic team meetings.
A high school diploma is required, while an associate's degree is preferred. Candidates should have basic computer skills and some knowledge of the Child Welfare System and family counseling.
Supplement case planner contacts in accordance with ACS minimum casework contact standards as directed. Assist with outreach and engagement efforts. Provide or arrange case coverage during workers' vacations or other leave. Continually assess family engagement, behavior changes, and in-home functioning. Advocate for and escort families to service sites, modeling appropriate behaviors and assessing learning transfer. Maintain collateral contact with service providers and request progress reports; share information with the case planning team. Actively participate in monthly case supervisions and clinical diagnostic team meetings. Aid in the development of comprehensive assessments. Ensure compliance with ACS training and program learning mandates. Participate in ACS and agency meetings, including FTCs, as directed. Ensure all written documentation and PROMIS contacts are entered accurately and promptly. Assist with maintaining physical case records. Perform additional duties as required to meet department/agency needs.
High School Diploma required; Associate's Degree preferred. Some knowledge of the Child Welfare System and experience counseling children and families. Basic computer skills. Strong organization and time management skills. Flexibility to meet changing needs and demands. Fluency in Spanish and English preferred. Ability to travel throughout the Bronx and to other CMCS locations.
