Position Details

Position: Part-Time LVN Location: Tice Valley Post Acute – Walnut Creek, CA Pay Rate: $35.00 per hour Why Join Tice Valley Post Acute? ✨ Competitive pay in a stable and rewarding healthcare environment ✨ Supportive nursing leadership and collaborative interdisciplinary team ✨ Opportunity for professional growth and continued learning ✨ Meaningful work caring for seniors and post-acute residents ✨ Convenient Walnut Creek location in the beautiful Tice Valley area Key Responsibilities Clinical Care Provide skilled nursing care including medication administration, treatments, wound care, and monitoring vital signs Deliver compassionate, resident-centered care focused on comfort, dignity, and safety Assessment & Monitoring Observe, assess, and document changes in resident condition Communicate concerns promptly to the RN Supervisor and healthcare providers Care Planning Assist with the development and implementation of individualized care plans Support resident recovery goals and quality-of-life initiatives Leadership & Team Support Supervise and support Certified Nursing Assistants (CNAs) Collaborate with interdisciplinary team members to ensure continuity of care Documentation & Compliance Maintain accurate clinical documentation and medical records Ensure compliance with all state, federal, and facility regulations and standards Qualifications ✔ Current and active California LVN license required ✔ Current BLS/CPR certification required ✔ Previous skilled nursing or post-acute care experience preferred ✔ Strong clinical judgment and communication skills ✔ Compassionate, dependable, and team-oriented approach to resident care Join a team that values your dedication, professionalism, and passion for patient care.
