About the role

Provide geriatric primary care and chronic disease management to residents through clinic, home, and assisted living visits. Work within an interdisciplinary team to deliver wellness and palliative care while managing acute issues.

Overview Dartmouth-Hitchcock - Kendal Clinic APRN or PA - Geriatrics The DH-Kendal Clinic located within the Continuing Care Retirement Community of Kendal at Hanover is seeking an enthusiastic and experienced APRN to join our team.

Our DH-Kendal Clinic provides a broad spectrum of geriatric primary care and chronic disease management.

Routine care is provided within clinic visits, independent living home visits, and assisted living Health Center visits.

Acute issues are also addressed urgently within these settings.

Emphasis is on wellness and palliative care for this unique and high functioning Kendal community of ~400 residents aged 65 and older.

The clinic provider team includes one physician and two nurse practitioners.

The ideal candidate will work intensively within an interdisciplinary team that includes nursing, social work, rehab, nutrition, and spiritual care.

Working within a team is a large component of this role so excellent and professional communication skills, with both patients and DH specialists, are critically important.

Autonomous clinical skill is important for periods of working alone.

On-call responsibilities are included in this role.

As the home to the Dartmouth Institute and Geisel School of Medicine, opportunities for academic appointment and advancement are rich for motivated providers.

Lebanon and the neighboring town of Hanover, home to Dartmouth College, are located in the “Upper Valley” which straddles the pristine Connecticut River, a national heritage river, and includes 46 towns in western New Hampshire and eastern Vermont.

This local region boasts excellent local schools, a myriad of cultural opportunities through Dartmouth College, as well as the Lebanon Opera House and Northern Stage in White River Junction, and an abundance of recreational opportunities.

Amenities associated with urban areas in Boston MA, Burlington VT, and Montreal QC are all within a few hours drive.

Qualifications

Graduate of an accredited APRN or PA academic program National specialty certification as FNP, WHNP, PNP, ANP or certification through the National Commission on Certification of Physician Assistants (NCCPA) Required Licensure/Certifications Eligible for RN licensure in the State of NH (APRN only) Eligible for APRN or PA licensure in the State of NH Eligible for Federal DEA Registration BLS certification

Requirements

Must be a graduate of an accredited APRN or PA program with national specialty certification (e.g., FNP, NCCPA). Requires eligibility for NH state licensure, Federal DEA registration, and BLS certification.
