Central Coast Home Health and Hospice is a locally owned organization with an excellent reputation throughout the San Luis Obispo and Santa Barbara counties. We are constantly growing and now seeking a Per Diem Hospice Admit Nurse to join our Hospice team in San Luis Obispo. The Admit Nurse completes an initial assessment of the patient to determine hospice needs, counsels the patient and their family, provides education as needed to the patient and family as related to their needs. This position requires accuracy in all aspects of the role and someone who is able to be flexible. Our nurses deliver high quality care to our hospice patients in their own home environment, collaborating with an interdisciplinary team to promote patient and family choice, assure continuity of care, and enhance the quality of life of dying patients. This highly rewarding field requires organization and the ability to work independently, managing your own caseload and making your own work schedule. Work schedule: Fridays, and one weekend day either Sat or Sun 8 AM-5 PM (flexible). ***NO NIGHT CALL Job Requirements: Current California RN License At least one year of professional nursing experience required CPR certified (Healthcare Provider) Reliable vehicle to drive for work Current Driver's license and clean driving record Have and maintain auto insurance coverage Knowledge/Abilities:
Hospice &/or Home Health experience is a plus
