The Registered Nurse provides home health services to help patients achieve the highest level of independence and well-being. They prepare patients and caregivers for a continuum of care within the home or community-based systems.
Candidates must be a graduate of an accredited nursing school with an active Colorado license and at least one year of RN experience. A valid driver's license and the ability to communicate in the client's language are required.
The goal of the home health Registered Nurse (RN) is to return the patient to the highest practicable level of independence and well-being within the community with self-management of disease and other identified processes. When the highest practicable level of well-being is not independence, the goal is to prepare the patient and family/caregivers for a continuum of care within the home and/or within other community-based health care systems.
