The Case Manager provides care coordination and crisis intervention for individuals with substance abuse and behavioral health disorders. Responsibilities include facilitating support groups, advocating for client services, and interfacing with agencies like DSS and the courts.
Candidates should possess a CASAC-T or CASAC certification and have at least one year of experience working with substance abuse and adolescent mental health populations. Strong organizational, networking, and collaborative skills are required.

Position: Case Manager
The Case Manager is responsible for the delivery of care coordination services to a population with history of substance abuse. The Case Manager serves as a member of the interdisciplinary treatment team.

This is a full-time or part-time non-exempt position.
This job operates in a professional office environment and outside in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must be able to travel among assigned sites and locations as needed.
