Provides direct and indirect patient care for adults 55+ with chronic conditions under the supervision of an RN. Responsibilities include monitoring health conditions, evaluating home care needs, and implementing individualized plans of care.
Requires a graduate degree from a vocational nursing school and an active California LVN license. Minimum of six months of LVN experience and BLS/First Aid certifications are required.
The LVN-PACE Program (Program for All-inclusive Care for the Elderly) works under the supervision of a Registered Nurse (RN) and provides developmentally appropriate and age-specific direct and indirect patient care for participants in the adult care program which serves adults 55 years of age or older with one or more chronic or post-acute medical, cognitive, or mental health conditions for whom a qualified healthcare provider has prescribed or requested services. Provides care in accordance with the established individualized plan of care for each participant based on diagnosis, orders, medical condition, psychosocial needs, physical condition, and other condition(s) pertinent to participant's specific situation. Additionally, this position will provide care in the place of the participant’s residence to evaluate home care needs, available support, and compliance with the developed Plan of Care. Seeks to improve or stabilize deterioration of the medical, cognitive and mental health condition. Focuses on participants' lives and monitoring healthcare conditions while providing needed healthcare. Utilizes medical knowledge and training to identify and determine observations and changes in conditions that are essential to be reported promptly to the RN and/or primary care physician. Facilitates cognitive stimulation and communicates with program participants in a friendly and congenial manner and treats them with dignity and respect. Performs designated duties in accordance with the LVN scope of practice and established policies, procedures, guidelines and techniques at both the LLUH PACE Center and in the place of the participant’s residence if needed. Applies knowledge base in the delivery of quality whole-person care and demonstrates behaviors which support excellence in customer service and satisfaction, patient safety and professionalism. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and participant population served. Performs other duties as needed.
Graduate of a school of vocational nursing. Minimum of six months of experience as an LVN required. Minimum one year of clinical experience in area of specialty preferred.
Active California Licensed Vocational Nurse (LVN) license required. Basic Life Support (BLS) certification and First Aid certification issued by the American Heart Association required.
