The Social Worker is responsible for managing the appropriate and timely discharge and transfer arrangements for patients referred to the Care Management Department. This includes coordinating clinical and social needs to ensure a safe transition from the hospital.
A Master's degree in social work and a current LMSW license from the Nevada State Board of Examiners are required. Candidates should have at least one year of hospital experience or a consultation plan with a qualified social worker.
