This is a full time, direct care position. Applicant must be a qualified professional (not licensed or provisionally licensed). The primary responsibilities of the Care Manager are to promote whole person, integrated care by planning, coordinating, tracking, closing of gaps in care and monitoring care to individuals to achieve the key goals of the behavioral health tailored plans. #ZR
