Provide personal care and support to hospice patients in their homes and care facilities following an interdisciplinary plan of care. Monitor patient conditions and maintain accurate documentation while collaborating with the nursing team.
Must be a state-certified home health aide or CNA with current CPR certification and a valid driver's license. Requires a reliable vehicle, auto insurance, and the ability to communicate effectively in English.
