Delivers direct/indirect patient care to adult acute inpatient rehabilitation patient utilizing the nursing process through data collection and assessment; implementation and prioritization of patient’s problems and needs; and development/implementation/evaluation of a nursing plan of care. Focuses on the unique needs of the rehab patient while transitioning them from an acute/post-acute care setting to a community setting.
Rehabilitation nurses are part of a interdisciplinary team and coordinate care with therapists, case managers, social workers, and a Physiatrist (Physical Medicine and Rehabilitation Physician) to assess the unique rehab needs that address deficits, implement a rehab nursing plan of care with comprehensive interventions and develop ongoing caregiver and family education, They must demonstrates critical thinking to assure the delivery of safe, quality, age specific care while delegating effectively to appropriate team members.
Rehab nurses are expected to document accurately and in a timely manner while utilizing the CARE tool as mandated by the Centers for Medicare/Medicaid Services. The rehab nurse is expected to adhere to organizational policies and procedures; regulatory accrediting body requirements; and professional practice standards.
