Requires an active Registered Nurse (RN) license in North Carolina and a minimum of two years of clinical nursing experience. Must possess current CPR certification and reliable transportation for home visits.

The Registered Nurse (RN) – Admission Assessment is responsible for conducting comprehensive pre-service, in-home clinical evaluations for prospective clients prior to the initiation of care.
This PRN position plays a critical role in establishing safe, appropriate, and individualized care plans based on a thorough assessment of the client’s medical condition, functional status, and home environment. The RN ensures compliance with regulatory standards, agency policies, and evidence-based clinical practice.
Perform comprehensive in-home admission assessments for new clients Obtain and review medical history, current diagnoses, medication profiles, and treatment plans Conduct physical, functional, cognitive, and psychosocial evaluations Assess home safety and environmental risk factors Identify skilled and non-skilled service needs Develop and document individualized care plans in accordance with physician orders and agency protocols Collaborate with interdisciplinary team members to ensure appropriate service coordination Ensure timely and accurate completion of documentation within established compliance standards Maintain adherence to federal, state, and local regulations governing home health services Protect patient confidentiality in accordance with HIPAA guidelines
Minimum Qualifications: Active, Registered Nurse (RN) license in the state of North Carolina
Minimum two (2) years of clinical nursing experience Demonstrated proficiency in patient assessment and care planning Strong clinical judgment and critical thinking skills Excellent written and verbal communication abilities Reliable transportation and ability to conduct home visits Current CPR certification
Prior home health, hospice, or community-based nursing experience Knowledge of state home care regulatory requirements
