Conduct comprehensive adult and geriatric assessments for hospice patients and manage their plans of care. Maintain accurate documentation and ensure continuity of care within a collaborative hospice environment.
Must hold current, unrestricted RN and Nurse Practitioner licensure in the state of practice. Board certification by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care is required.
Proactive, collaborative team member in a hospice care environment Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care Commitment to caring for patients and partners Complete all documentation and records in a timely and accurate manner Follow established policies and procedures as related to the scope of practice Maintain continuity of patient care in a hospice setting
