The Care Coordinator RN provides specialized nursing care for patients with complex congenital heart disease, managing admission and discharge planning. They collaborate with a multidisciplinary team to provide chronic care management, telephone triage, and patient education.
A BSN and a minimum of 2 years of pediatric cardiac clinical experience are required. Candidates must also hold a BLS certification from the American Heart Association.
Nemours is seeking a Care Coordinator RN for our Cardiology team in Jacksonville, FL.
The Cardiac Center High Risk Nurse (RN) Coordinator is responsible for providing specialized professional nursing care to patients with complex congenital heart disease by providing direct and indirect care in collaboration with the physicians, advanced practice providers, nursing, social workers, genetic counselors, nutrition services and other clinical staff including but not limited to admission and discharge planning of all patients admitted to the comprehensive cardiac care unit. This position will work in a multidisciplinary team to innovate, improve, and advance the quality of care through a patient centered approach.
