The specialist provides pharmaceutical care focused on glycemic management and improving HCHAP medication scores. They are also responsible for training physicians and pharmacists while participating in quality improvement teams and formulary maintenance.

Education Doctor of Pharmacy (Pharm.D.) Post Graduate Training PGY1 or equivalent experience as clinical pharmacist PGY2 in specialty or equivalent experience Licensure/Certification/Registration Valid Florida Pharmacist License Maintain Board Certification in specialty from the Board of Pharmacy Specialties (BPS) or equivalent certification if not offered by BPS Special Skills/Qualifications/Additional Training/Experience Required Must be able to read, write, speak and understand English. Attend at least one nationally recognized meeting in area of specialty each year Maintain current membership in organization dedicated to area of specialty
Candidates must hold a Doctor of Pharmacy (Pharm.D.) degree and have completed PGY1 and PGY2 residency training or equivalent experience. A valid Florida Pharmacist License and Board Certification in a specialty (BPS) are required.
