The HIM Clerk manages the creation, maintenance, and breakdown of patient medical records in a psychiatric hospital setting. Responsibilities include charting documents, managing forms, and providing clerical support to the Health Information Management department.
A high school diploma or GED is required, with a preference for knowledge of medical and psychiatric terminology. Candidates must be proficient with office software and available for a mandatory 8-day full-time orientation.
1. HS Dploma/GED required. 2. Knowledge of both medical and psychiatric terminology preferred. 3. Must demonstrate familiarity with common office machines and standard software applications. 4. Must possess strong communication skills, including verbal and written, as well as be able to effectively multi-task. 5. Strong organization, computer, and technology skills MUST HAVE availability to attend an 8- business day Orientation from 8a-4p without interruptions. Answers the phone, takes and delivers messages to the appropriate individuals in a courteous and professional manner. 2. Initiates the medical record at time of admission (discharge or transfer). 3. Maintains the approved medical record format in binders; assures that the tab dividers are appropriately placed and that appropriate documentation is behind the correct tab divider. 4. Charts all loose documents, i.e., labs, diagnostic reports, transcribed reports, and any other “to be filed” documentation into patients’ medical records. 5. Ensures that there is an ample supply of all forms utilized in the medical record; orders forms when levels are low. 6. Ensures that adequate blank forms such as Physician Orders, Progress Notes, etc., are available in the chart for use by physicians and other clinicians, social workers, etc. 7. Reduces amount of documentation in charts according to policy and sends to HIM department with discharges. 8. Breaks down medical record at time of discharge, maintaining approved medical record format, and delivers to HIM Department. 9. Under the supervision and direction of the RNs/LPNs on the unit(s), assists with clinically related activities to include but not limited to remaining in patient areas whenever requested to assist with patients and visitors. 10. Maintains and adheres to facility employee attendance guidelines 11. Understands and demonstrates knowledge of Treatment of Care between acute, residential, and Group Home (age specific), and attends all mandatory unit meetings discussing Treatment of Care. 12. Demonstrates knowledge and understanding of National Patient Safety Goals, and the policies and standards of accreditation entities including but not limited to Joint Commission, DMH, DHR, and CMS. 13. Performs other duties as assigned/required by this position.
