Responsible for preparing, organizing, and maintaining both paper and electronic medical records for patient visits. This includes processing document imports, handling release of information inquiries, and tracking provider documentation deficiencies.
Requires a high school diploma or GED and at least three years of office experience, preferably in a medical setting. Knowledge of electronic health record systems is required, while medical terminology coursework is preferred.
Scans or electronically processes documents and prepares documents for importation into the electronic document management system. Import documents into the electronic medical record according to established guidelines. Distributes and processes electronic faxes according to established guidelines. Tracks provider documentation deficiencies as per established guidelines. Performs data entry functions relevant to the needs of the organization. Prepare charts for scheduled appointments in advance according to guidelines. Copies, mails, and/or faxes patient chart information as requested and authorized. documents all disclosures as required. Maintains both paper and electronic patient medical record. Assists in archiving deceased and inactive charts as scheduled and according to guidelines. Maintains communication with internal and external customers relevant to the position. Keeps inventory and orders office supplies relevant to the department as Demonstrates an understanding of patient confidentiality to protect the patient and clinic/corporation. Follows policies and procedures to contribute to the efficiency of the HIM Department. Covers for other HIM Department functions as requested. Performs all other duties/responsibilities as necessary or assigned.
Formal Education, Certification/Licensure, and Prior Experience: to perform this job successfully, an individual must have: a High School Diploma or a General Education Degree a minimum of three (3) years of office experience (preferably in a medical office) Knowledge of electronic health record systems. Time Management, Organization, Attention to Detail and Quality Focus skills needed. It is preferred that an individual has: previous experience in a medical office setting, preferably in an HIM department taken a medical terminology course Physical Demands: Regularly: sit, grasp, reach, bend, stoop/crouch, talk, and hear Occasionally: stand, walk, lift up to 30 pounds, pull/push up to 30 pounds, and carry up to 30 pounds Vision: clearly see up to 5 feet with or without corrective lenses. Work Environment: While performing the essential duties/responsibilities of this job, the employee will not be exposed to any workplace environmental conditions caused by extreme noise, heat, cold, workplace hazards, atmospheric dust/fumes/vapors/mists, pathogens, or toxic chemicals.
