The LPN is responsible for administering medications, performing clinical treatments, and managing resident care plans. Additionally, the role involves supervising nursing assistants and participating in Quality Assurance and Performance Improvement (QAPI) efforts.
Candidates must be a Licensed Practical Nurse or board eligible in the Commonwealth of Pennsylvania. Required clearances include Act 169 and Act 13 criminal history background checks.
45pm - 11:15pm. Hours are on Saturdays & Sundays and possibly other days, if available. Preferably experienced long term care and/or rehabilitation nurse.. Position will include at least one spring/summer holiday and one fall/winter holiday.
1. Performs treatments, chemstrips, obtains I & O, and performs licensed nursing duties (catheterization, enemas, suction, tube feeds, etc…).
2. Administers medications and participates in annual medication pass assessment
3. Performs treatments and communicates effectively in both verbal (physician, resident, guests, and staff) and written prose (charts, care plans, cardex, etc…).
4. Evaluates resident needs, condition, history, and care. Orients, admits, discharges, and transfers residents.
5. Keeps resident care plan updated and assures that the nursing staff is adhering to that plan.
6. Supervises, guides, and orients nursing assistants. May assume lead of unit.
7. Adheres to nursing home’ state and federal regulations.
8. Recognize your role as part of the QAPI efforts of your organization.
9. Attend trainings to build understanding and capacity to undertake QAPI work.
10. Carry out QAPI role and responsibilities as assigned.
11. Follow established policies and procedures in support of QAPI efforts.
12. Look for and share with leadership ideas for improvement in the organization.
13. Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success.
14. Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings.
Promotes a culture of safety, empowerment and healing; recognizes that everyone who interacts at Rest Haven may have past traumatic experience and protects trauma survivors from being re-traumatized.
Participates in annual compliance and ethics training. Follows code of conduct reporting any compliance and ethics concerns.
1. Assists with physical and occupational therapy.
reading, writing letters, operating TV/radio, and running in-house errands.
3. Performs assigned clerical duties at nursing station. Answers telephone and intercom. Runs errands as directed by supervisor.
4. Participates in unit cleaning duties, such as cleaning of wheelchairs and gerichairs, which are not or have not been performed by housekeeping personnel, as directed.
5. May perform other duties as directed by supervisor.

3. Sits, stands, and walks intermittently throughout the workday. Bends, stoops, lifts, pushes, pulls, etc…
1. Licensed practical nurse or board eligible in the Commonwealth of Pennsylvania.
2. Professional, communicator, team builder, leader, organized, and a problem solver.
3. Knowledge of government nursing home regulations preferred.
