Baptist Health is looking for a Registered Nurse, Case Manager at Baptist Medical Jacksonville - Downtown campus.
This is a PRN Day shift opportunity working Monday - Friday in the In-Patient Case Management Department at Baptist Jacksonville - Downtown campus.
As a Case Manager, you are responsible for: Care coordination for hospitalized patients.
High risk screenings and assessments.
Coordinating post-acute care services with proper documentation.
Collaborating with multidisciplinary team members.
