$22 - $25 / HOUR
Provide direct care services to children and families, including assessment, care planning, and crisis intervention through monthly face-to-face visits. Coordinate medical, social, and educational needs while collaborating with providers to ensure optimal healthy development.
Requires a BSW with 3 years of experience or an MSW with 2 years of experience working with children with special needs. Must have at least 2 years of experience with severely disabled populations and a valid Tri-state driver's license.

The Harmony Healthcare Long Island is a non-profit healthcare organization with 7 health centers, providing primary care and preventative medicine in the following locations in Nassau County: Roosevelt, Elmont, Hempstead, Freeport, Oceanside, and New Cassel/Westbury. In addition, the Harmony Healthcare Long Island has 4 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program.
