The Community Health RN provides direct clinical support for mobile health services to vulnerable populations. They are responsible for identifying social barriers to health and developing community improvement programming based on local needs assessments.
Candidates must have an ASN, a valid Florida RN license, and BLS certification. A minimum of 2 years of RN experience is required, with a preference for experience working with vulnerable populations.
The Community Health RN provides direct community health services through Care Connect to vulnerable populations through identification, engagement, and provision of resources to address unmet social and health needs.
