Essential Functions Conduct comprehensive psychosocial assessments to determine patient risk levels and needs Participate in the development and implementation of individualized plans of care Plan, arrange, implement, and evaluate psychosocial programs for patients and families Develop and deliver education and counseling programs for individuals and groups Serve as a liaison between patients, families, community organizations, and healthcare providers Provide short‑term counseling and crisis intervention as needed Identify and address psychosocial disorders, interpreting patient needs for professional and lay audiences Support community and social programs in alignment with federal, state, and local grant or contract requirements Prepare quarterly and ad‑hoc reports as required Collaborate with clinical, health education, and community health staff to ensure coordinated care Provide appropriate referrals and follow‑up for needed services Utilize assessment tools, measurement scales, and objective criteria to evaluate patient needs and outcomes Deliver in‑service trainings and presentations for staff as needed Conduct community outreach and provide health‑related educational presentations Perform additional responsibilities as assigned
