The Speech Language Pathologist will conduct home visits to assess, diagnose, and provide therapy for speech, language, communication, or swallowing difficulties, developing and revising individualized treatment plans based on evaluations and physician orders. Responsibilities also include educating patients and families on rehabilitative care and accurately documenting all interventions and observations using a tablet.
Candidates must possess a current Speech Language Pathologist license in New Hampshire and hold or be in the process of obtaining the Certificate of Clinical Competence from ASHA, which includes meeting specific educational and work experience requirements. Solid computer skills are necessary, with prior experience using Electronic Medical Records preferred.
