Supports hospital discharge planning for inpatient patients to ensure a safe and cost-effective transition to the appropriate level of care. Facilitates the continuum of care and the overall patient through-put process.
A high school diploma or equivalent is preferred, along with previous experience in clinic work or insurance knowledge. Candidates must possess strong computer skills and the ability to work independently to meet deadlines.
