The Home Health Licensed Practical Nurse (LPN) provides skilled nursing care with the goal of restoring, maintaining and promoting optimal mental and physical health of patients through assessment, teaching, counseling and appropriate preventative and rehabilitative nursing measures.
Conduct on-going assessments of the patient’s physical, psychological and social needs.
Include the patient’s family and/or significant other, when indicated, in the assessment of the patient’s needs.
Ensure documentation meets reimbursement criteria and adhere to agency policies and guidelines.
Communicate changes in condition/needs in a timely manner to appropriate agency personnel, physicians, patient’s family/caregiver, and outside resources as indicated.
Communicate and collaborate with members of the health care team through case conferences to meet patient needs, when indicated.
Demonstrate knowledge of community resources and utilize them appropriately.
Plan individualized patient care utilizing professional knowledge.
Organize and analyze data collected to identify patient needs.
All other duties as assigned.

Active Virginia (or compact) unencumbered LPN License.
At least one-year experience in acute clinical care.
Ability to meet deadlines while working under limited supervision.
Current CPR certification.
Valid state driver’s license.
Motor Vehicle Driving Record satisfactory to Westminster-Canterbury and its insurance providers.
Proof of motor vehicle insurance.
Flu-vaccinated
