Delivers coordinated nursing care to patients in their residences and supervises assigned nursing staff. Coordinates care with other disciplines and manages patient assessment, planning, and evaluation.
Requires a Registered Nurse license in Oregon (and Washington within 90 days), CPR/BLS certification, and at least one year of professional nursing experience. A Bachelor's Degree in Nursing is preferred.
