A Brief Overview The Home Care Clinical Nurse is responsible for providing nursing care and services to home care patients in accordance with the CHOP, CHHC standards of practice, policy and procedure and within all regulatory and accrediting agency guidelines. Utilizes the nursing process to deliver nursing care to patients and families.

• Practice: Provides competent care for patients and families based on experience. Nursing practice and documentation is in accordance with CHOP, CHHC standards of care, policy & procedure as well as all regulatory and accrediting agency guidelines. Participates in on
• call rotation to meet patient care needs.
• Professionalism/Leadership: Demonstrates competence in professional practice.

Performs other duties as assigned by Home Care Management. Education Qualifications Technical Diploma Nursing - Required Bachelor's Degree Nursing - Preferred Experience Qualifications Pediatric or Adult acute, subacute, other inpatient or Home Care experience - Required At least two (2) years of inpatient nursing with pediatrics/adults - Preferred At least one (1) year of home care experience - Preferred Skills and Abilities Ability to operate a company vehicle and/or be insured on a CHOP policy. (Required proficiency) Strong interpersonal and customer service skills (Required proficiency) Strong verbal and communication skills (Required proficiency) Strong clinical judgement skills (Required proficiency) Ability to work independently (Required proficiency) Ability to work as a team player in a fast paced environment (Required proficiency) Data entry skills (Required proficiency) Licenses and Certifications Driver's License - Department of Motor Vehicles - upon hire - Required and Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required and Registered Nurse (New Jersey) - New Jersey State Licensing Board - within 9 months - Required To carry out its mission, CHOP is committed to supporting the health of our patients, families, workforce, and global community. • RANGE: $43.19 - $55.07 Hourly Salary ranges are shown for full-time jobs.
