The Hospice LPN provides direct patient care under the supervision of a registered nurse, following established plans of care. They work collaboratively with an interdisciplinary team to achieve positive patient outcomes for families facing end-of-life care.
Candidates must hold a current Georgia LPN license and have at least two years of experience, preferably in acute, oncology, home health, or hospice settings. A valid driver's license and reliable transportation are also required.
