The Hospice Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

• EDUCATIONAL REQUIREMENTS Possess a current license to practice as an LPN in the state of Georgia Graduate of an accredited practical nursing program EXPERIENCE REQUIREMENTS Minimum of two (2) years experience as an LPN and preferably one of the two years in an acute, oncology, home health, and/or hospice setting.
• Knowledge of the Medicare Certified Hospice Program preferred.
• OTHER REQUIREMENTS Possess a valid driver’s license in the state of Georgia.
• Ability to either drive or have available reliable transportation and present evidence of valid automobile liability insurance.
• Ability to communicate effectively, orally and in writing, and to comprehend written and verbal communication.
• Ability to operate computer equipment, fax, and copier machines.
• Ability to work a flexible schedule.
• Ability to work well within an interdisciplinary team setting as well as independently.
• Ability to work with patients and families dealing with life-threatening illness and death.
• Ability to assess and respond to the needs of patients and families in varied settings.
• Demonstrates skill in nursing practice, ability to cope with family’s emotional stress, and tolerance of individual lifestyles.
• Ability to make meaningful observations and write comprehensive accurate reports.
• Demonstrates excellence in managing human resources.
• Demonstrates effective public speaking skills and the ability to develop good interpersonal relationships.
