The Home Infusion Staff Pharmacist will assist in managing pharmacy operations and provide clinical care to patients. Responsibilities include evaluating patient therapies, monitoring drug interactions, and ensuring compliance with pharmacy policies.
Candidates must have a B.S./B.A. or Pharm D in Pharmacy and at least one year of experience as a practicing pharmacist. A Minnesota Pharmacy License is also required.
Assist the pharmacy manager in the operation and management of the pharmacy, provide clinical care to patients of the pharmacy, provide indirect supervision for the support staff and ensure adherence to all policies and procedures.
Perform initial patient evaluation as it pertains to the prescribed therapies. Collect, analyze, interpret, and evaluate appropriate data relative to medication therapy. Obtain histories and apply these along with other patient data to optimize drug therapy management. Work with other healthcare providers to evaluate patients to determine appropriateness for home IV therapy. Evaluate the significance of signs and symptoms relative to disease processes and makes recommendations for the modification of drug therapy. Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy. Monitor patient records for potential drug/drug or drug/food interactions as well as patient allergies. Provide counseling and answer questions from patients, family and health care professionals concerning drug therapy. Provide prescription drug therapy to patients/customers ensuring accurate labeling and dispensing of medications in a timely fashion. Become familiar with Home Care and Pharmacy billing process. Evaluate patient supply of medications and IV access supplies. Assist patient and other healthcare providers on maintaining IV line for administration of medication. Monitor and evaluate clinical outcomes. Ensure dispensing of prescriptions in accordance with all legal requirements, regulatory agency and Fairview Home Infusion Policy and Procedures. Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences.
Required Qualifications High School or GED B.S./B.A. or Pharm D in Pharmacy from an accredited school of pharmacy 1 year experience as a Practicing Pharmacist MN Pharmacy License
