JOB REQUIREMENTS • Licenses

Active unrestricted Indiana Medical License; Indiana CSR; DEA with IN address and schedules 2, 2N, 3, 3N, 4
Board: Board Eligible or Board Certified in Emergency Medicine through ABMS/AOA
DEA: Active DEA required
Certifications:
BE in EM: ACLS, ATLS, BLS, PALS required
BC in EM: ACLS, BLS, PALS required and proof of having ATLS at least once
Experience: Completed Emergency Medicine residency
Other
Qualifications:
Documentation of case logs for the past 2 years
CME Attestation Form completion during application process
Current Flu, TB, COVID immunization documentation
