The Social Worker performs psychosocial assessments and collaborates with a multidisciplinary team to develop discharge plans for high-risk patients. They serve as a liaison between the hospital and community agencies to ensure patients achieve their health goals.
Candidates must hold a Master's degree in Social Work and a valid NY State LMSW or LCSW license. Experience in patient discharge planning and knowledge of healthcare legal aspects are required.
The Social Worker functions as a part of the multidisciplinary care team to plan, organize, and implement social work services for patients and families based on individualized needs. The Social Worker performs these functions consistent with the Utilization Review and Quality Improvement Plans with a patient-focused function to promote the restoration of health and the achievement of patient health goals.
