Responsibilities

• Perform psychosocial assessment of Social Work High-Risk Criteria cases. Assess and assist patients and families to identify discharge barriers. Work with an interdisciplinary team to develop a discharge plan for complicated psychosocial issues that are identified. Provide information and referrals based on community resources and within patient insurance coverage. Serve as a liaison between community agencies and hospital. Supervision of profession social work students when appropriate. All other duties assigned. REQUIRED EXPERIENCE AND QUALIFICATIONS: NY State Licensed in Social Work required LMSW or LCSW. Master's degree in social work. Experience in patient discharge planning. Knowledge of legal aspects of healthcare. Excellent communication skills. As part of employment at Southampton Hospital, there may be occasions when travel to other locations is needed to support operational needs.
