A Brief Overview The Home Care Clinical Nurse is responsible for providing nursing care and services to home care patients in accordance with the CHOP, CHHC standards of practice, policy and procedure and within all regulatory and accrediting agency guidelines. Utilizes the nursing process to deliver nursing care to patients and families.

What you will do Practice

• Provides competent care for patients and families based on experience. Nursing practice and documentation is in accordance with CHOP, CHHC standards of care, policy & procedure as well as all regulatory and accrediting agency guidelines. Participates in on
• call rotation to meet patient care needs.
• Professionalism/Leadership: Demonstrates competence in professional practice.

• Demonstrates an awareness of research and its value to clinical practice. Participates in efforts to apply research findings to practice.

Performs other duties as assigned by Home Care Management. Education Qualifications Technical Diploma Nursing - Required Bachelor's Degree Nursing - Preferred Experience Qualifications Pediatric or Adult acute, subacute, other inpatient or Home Care experience - Required At least two (2) years of inpatient nursing with pediatrics/adults - Preferred At least one (1) year of home care experience - Preferred Skills and Abilities Ability to operate a company vehicle and/or be insured on a CHOP policy.
