About the role

The APP will provide high-quality, patient-centered primary care for patients of all ages in an outpatient setting. Key duties include diagnosing acute and chronic conditions, managing treatment plans, and coordinating care with a multidisciplinary team.

Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join Family Practice Center of Westerville. The APP will work collaboratively with physicians and clinical staff to provide high-quality, patient-centered primary care for patients of all ages. Responsibilities include assessment, diagnosis, treatment planning, chronic disease management, preventive care, and patient education in a busy outpatient family medicine setting.

The practice emphasizes preventive medicine, chronic disease management, patient education, and long-term wellness. The office includes:
  • Four experienced family medicine physicians
  • A collaborative and supportive clinical team
  • Dedicated medical assistants and patient service representatives
  • An established patient population and strong community reputation

Office hours

  • Monday -
  • Thursday: 7:00 AM – 4:00 PM
  • Friday: 7:00 AM – 3:30 PM
  • Key Responsibilities
  • Perform comprehensive patient evaluations including medical histories, physical examinations, and diagnostic assessments
  • Diagnose and manage acute and chronic medical conditions across the lifespan
  • Provide preventive care services including annual wellness visits, screenings, immunizations, and health counseling
  • Develop and manage treatment plans for chronic diseases such as diabetes, hypertension, asthma, COPD, and hyperlipidemia
  • Prescribe medications and monitor treatment effectiveness in accordance with Ohio licensure and practice guidelines
  • Educate patients and families on disease prevention, treatment options, medications, and healthy lifestyle strategies
  • Collaborate with physicians, nursing staff, and specialists to coordinate comprehensive patient care
  • Facilitate referrals, follow-up care, and transitions of care as needed
  • Maintain accurate and timely documentation within the electronic health record system (EPIC)

Qualifications

  • Master’s degree from an accredited Nurse Practitioner or Physician Assistant program.
  • Current Ohio licensure as a Nurse Practitioner or Physician Assistant.
  • National certification as a CNP or PA-C.
  • Minimum of one year of post-graduate clinical experience preferred, ideally in outpatient primary care or family medicine.
  • Strong clinical assessment, diagnostic, and patient management skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
  • Experience with EPIC EHR preferred, though training will be provided.

Requirements

Candidates must hold a Master's degree from an accredited NP or PA program and possess current Ohio licensure and national certification. A minimum of one year of post-graduate clinical experience in primary care is preferred.
