Monday-Friday from 8:30am-5:00pm. This role also has the potential to becoming Hybrid in the future! The RN will work collaboratively with physicians, advanced practitioners, and other healthcare professionals to deliver patient-centered care, focusing on health promotion, disease prevention, and the management of acute and chronic conditions. This role is primarily phone triage on the phone with patients as well as some patient facing duties, so the ideal candidate profile is an adaptable RN who is confident in their clinical abilities and genuinely enjoys variety!
Responsibilities
Include: - Triage clinical questions via phone calls. - Provide clinical advice and education to patients based on assessment findings and established protocols. - Provide appropriate care coordination for patients such as scheduling appointments, follow-up care and referrals to specialists and other healthcare services. - Follow-up on emergency room visits. - Collaborate with physicians, advanced practitioners, and other healthcare team members to ensure comprehensive patient care. - Collaborate with healthcare team members to develop and implement individualized care plans. - Provide direct patient care, including assessments, planning, implementation, and evaluation of care plans. - Administer Injections, medications, treatments, and procedures as prescribed by physicians and advanced practitioners and assist in minor surgical procedures.
