The Physical Therapist provides care and services to home health patients with various impairments in their residences or care facilities. They collaborate with an interdisciplinary team to evaluate and treat neurological, musculoskeletal, and cardiopulmonary disorders.
Candidates must hold a current Physical Therapist license in good standing with at least one year of experience. Additional requirements include CPR certification, a valid driver's license, and reliable transportation.

This position is a contributing member of the interdisciplinary team and evaluates and services to the home health patient with neurological, musculoskeletal, cardiopulmonary impairments; gross motor, sensorimotor, and developmental delays; as well as traumatic injuries and orthopedic disorders.
Current Physical Therapist licensure (in good standing) in the state(s) of practice with one year of experience under license Ability to work well within a team environment to collaborate daily with a team of clinicians Work closely with patients on a 1-1 level but get strong support from a patient care team Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of state and federal rules and regulations Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance CPR Certification
