Req DEA Certificate Unrestricted DEA Certificate with a NV address.
Req Specialty Certification Board certified with current certificate in specialty area.
Req Basic Life Support (BLS) Healthcare Provider from American Heart Association
Req Board of Pharmacy License Board of Pharmacy - Valid Nevada Board of Pharmacy License
Req Certification - Job Relevant Must obtain USC Credentialing from governing board within 120 Days of hire.
Req Fire Life Safety Training (LA City) If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)
The annual base salary range for this position is $191,360.00 - $315,744.00.
