Provide direct patient care and education within the chronic dialysis unit and support patients transitioning to home therapy. Collaborate with interdisciplinary teams to develop individualized care plans and maintain strict regulatory documentation.
Requires a Bachelor's Degree in Nursing, a current New York State RN license, and BCLS certification. Preferred candidates possess a Master's degree, specialized dialysis certifications, and extensive experience in both acute and chronic dialysis care.
