About the Role

As a Consonus Physical Therapist, you’ll deliver person-centered, evidence-based care to highly motivated patients—many of them seniors—helping them regain function and independence. You’ll be part of a collaborative team, using advanced clinical pathways to guide therapy plans and elevate outcomes. Key Responsibilities: Design and implement personalized therapy programs using clinical data and Consonus’ proven pathways. Help patients maximize physical, emotional, social, and cognitive function. Qualifications

• Graduate of an accredited program with a degree in Physical Therapy Current state licensure
• or- in the process of obtaining appropriate state licensure CPR Certification (requirement varies based on State/location) National Provider Identifier (NPI) required upon hire Excellent organizational skills to facilitate program development Strives to positively represent their discipline and Consonus Rehab in the community New Grads encouraged to apply – grow your skills and experience with us!

Salary Range
