  • Purpose: The Behavioral Health Registered Nurse (RN) is a highly qualified health care provider who, through education and experience, possesses a distinct body of knowledge and skills and applies the art and science of his/her profession.
  • Scope &
  • Objective: Applies specialized psychiatric nursing knowledge in assessment, diagnosis, and treatment of patients with behavioral health concerns. Assists the Lead Behavioral Health RN in building and sustaining the behavioral health program for patients receiving care in their homes or in assisted living facilities. Functions under the direction of the Nursing Supervisor and the Director of Patient Clinical Services (DPCS). Initiates case findings, health teachings health counseling and provision of care that supports life or restores well-being.

  • Demands: While performing the duties of this job, the employee is regularly required to talk and hear. The position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift or move items weighing 50 pounds.

Travel

This position requires daily travel within the front Range of Colorado.

Responsibilities: Complies with accepted ethical conduct and Professional Standards of Nursing Practice as set forth by the American Nurses Association Practices and maintains patient safety and dignity by abiding by Federal, State, Local and Optimal policy & procedures as pertains to patient protected health information and HIPAA (Health Information Portability and Accountability Act). Assesses client’s condition and needs, through the collection of health data, the recognition, interpretation and reporting of symptoms and psychological responses and re-evaluates the client nursing needs as required Develops and implements nursing plan of care with consideration for the dignity and desires of the client care, organizes and participates in direct client care, performs treatment and administers medications Formulates, notifies, and carries out client care plan which prescribes nursing actions toward achievable goals according to client needs Maintains a safe, therapeutic environment for the client Initiates and participates in the development of the Plan of Treatment, obtains physician’s orders, clarifies those orders and obtains necessary revisions as needed, informs the physician of changes in the clients condition and provides written summary reports to the physician Participates in health counseling and teaching with the client and of family members Maintains detailed records of nursing actions, client progress, and response to therapy and teaching so that the Plan of Care can be documented and evaluated. Records clinical/progress notes in the client’s clinical record. Supervises care given by ancillary personnel. Initiates preventative and rehabilitative nursing procedures. Coordinates discharge planning and prepares discharge summaries and instructions. Communicates information effectively to appropriate personnel to facilitate continuity of care Maintains competency by participating in in-service and continuing educational programs Participates in Monthly, Quarterly, Annual trainings as well as Zone meetings Coordinates Nursing, Home Health Aide, Physical Therapy, Occupational Therapy, Speech Therapy and other services delivered to clients of the agency This list does not serve as an official job description of the complete list of duties. **Optimal compensates employees based on a per visit rate.*

QualificationsQualifications: Minimum of 2 years of experience in Psychiatric Inpatient, Outpatient or Home Care treatment setting Must be currently licensed and in good standing in the state of Colorado. Must be able to communicate in the language of the client and document client related activity as appropriate. Possesses adequate and effective written and verbal communication skills. Possess adequate and effective interpersonal skills.

Requirements

Candidates must have at least 2 years of experience in psychiatric inpatient, outpatient, or home care settings. A current nursing license in good standing in the state of Colorado is required.
