Building: Develop and maintain supportive, ongoing relationships with young clients and their caregivers using strengths-based interventions.
Treatment
Planning: Facilitate initial treatment planning meetings and 90-day reviews. Partner closely with therapists to update goals, objectives, and services as clinically indicated.

Coaching &
Advocacy: Assess family strengths and needs, interpret service options, and provide family coaching, education, and community service navigation.
Crisis &
Safety
Support: Coordinate crisis intervention services in collaboration with care team members, maintaining an acute awareness of safety risks and appropriate interventions. 2.

Collaboration &
Administration
Teamwork: Collaborate with multi-disciplinary teams and external partners, including physicians, schools, therapists, and community providers.
Waiver
Management: Update Plans of Care for Severe Emotional Disturbance (SED) waiver patients (30 days post-start, and every 90 days thereafter) and coordinate with Utilization Review for service authorizations.
Timely
Documentation: Maintain exceptionally accurate records. Our team standards include completing 90% of progress notes within 72 hours, completing waiver documents/budgets within two weeks of a case conference, and submitting service verifications within 72 hours. 3.
